Thanksgiving Meal Assistance Acknowledgment

The Lapointe Foundation is proud to support local families during the Thanksgiving season by providing a complete Thanksgiving meal for selected families, regardless of family size.

Due to the anticipated high volume of applications and the availability of donations, sponsorships, and resources, submitting an application does not guarantee that your family will receive a Thanksgiving meal. We will make every effort to assist as many families as possible, and families selected to receive a meal will be contacted with additional details.

By checking the box below, I acknowledge that I understand submitting this application does not guarantee that my family will be selected to receive a Thanksgiving meal from The Lapointe Foundation.

Photo & Media Release

I, the undersigned parent or legal guardian, give The Lapointe Foundation permission to photograph and/or record my child and/or family during the Thanksgiving event.

I understand that photographs and recordings may be used by

The Lapointe Foundation for nonprofit and promotional purposes, including social media, the Foundation’s website, newsletters, printed materials, and other communications intended to highlight our programs and community efforts.

I understand that participation is voluntary and that no compensation will be provided for the use of these photographs or recordings. I also understand that The Lapointe Foundation will make reasonable efforts to use images respectfully and appropriately.

By checking the box below, I confirm that I have read and understand this Photo & Media Release and give my permission for the use of photographs and/or recordings as described above.

Liability Waiver & Event Participation Agreement

I, the undersigned parent or legal guardian, give permission for my child and/or family to participate in the Thanksgiving event hosted by The Lapointe Foundation.

I understand that participation in community events and activities may involve certain risks, including but not limited to slips, falls, minor injuries, or other unforeseen incidents. I acknowledge these risks and agree to assume responsibility for my child’s and/or family’s participation in the event.

To the fullest extent permitted by law, I agree to release and hold harmless The Lapointe Foundation, its directors, officers, employees, volunteers, sponsors, and event partners from claims arising from my child’s or family’s participation in the event, except to the extent caused by their gross negligence or intentional misconduct.

I understand that The Lapointe Foundation is not responsible for personal belongings that may be lost, damaged, or misplaced during the event.
