Liability Waiver & Parental Consent
I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the Youth Flag Football Event hosted by The Lapointe Foundation. I acknowledge and understand that participation in sports activities involves inherent risks, including but not limited to physical injury.
In consideration of my child being allowed to participate, I hereby release, waive, and hold harmless The Lapointe Foundation, its organizers, volunteers, staff, and affiliates from any and all liability, claims, demands, or causes of action that may arise from or in connection with my child’s participation in this event, including any injury, illness, or accident.
I confirm that my child is physically able to participate in this activity and I assume full responsibility for any medical expenses incurred as a result of injury sustained during participation. I also grant permission for emergency medical treatment if deemed necessary.
