Acknowledgement

The Lapointe Foundation is committed to helping local families experience a little extra joy this holiday season. Due to the anticipated high volume of applications, submitting an application does not guarantee that a family will be selected to receive assistance.

We will make every effort to support as many families as possible based on the sponsorships, donations, and resources available to us. Families who are selected will be contacted with additional information regarding their assistance.

By submitting this application, I acknowledge that completing an application does not guarantee that my family will receive Christmas assistance and that selections will be made based on the resources available to The Lapointe Foundation.

Liability Waiver & Event Participation Agreement

I, the undersigned parent or legal guardian, give permission for my child and/or family to participate in the Christmas event hosted by The Lapointe Foundation.

I understand that participation in community events and activities may involve certain risks, including but not limited to slips, falls, minor injuries, or other unforeseen incidents. I acknowledge these risks and agree to assume responsibility for my child’s and/or family’s participation in the event.

To the fullest extent permitted by law, I agree to release and hold harmless The Lapointe Foundation, its directors, officers, employees, volunteers, sponsors, and event partners from claims arising from my child’s or family’s participation in the event, except to the extent caused by their gross negligence or intentional misconduct.

I understand that The Lapointe Foundation is not responsible for personal belongings that may be lost, damaged, or misplaced during the event.

Photo & Media Release

I, the undersigned parent or legal guardian, give The Lapointe Foundation permission to photograph and/or record my child and/or family during the Christmas event.

I understand that photographs and recordings may be used by The Lapointe Foundation for nonprofit and promotional purposes, including social media, the Foundation’s website, newsletters, printed materials, and other communications intended to highlight our programs and community efforts.

I understand that participation is voluntary and that no compensation will be provided for the use of these photographs or recordings. I also understand that The Lapointe Foundation will make reasonable efforts to use images respectfully and appropriately.
